You’re planning your move from the U.S. to Spain, so health insurance naturally makes its way onto your checklist.
You start researching international health insurance and find what looks like a great option: worldwide coverage, access to healthcare in multiple countries, protection while traveling—and Spain is included.
It sounds perfect. After all, if your policy covers you while you’re living in Spain, surely it should also work for your Spanish visa.
But there’s one more question you need to ask:
“Does your specific policy meet the health insurance requirements of the visa you’re applying for?”
Because international coverage and visa compliance are not necessarily the same thing.
“Covered in Spain” and “Visa-Compliant in Spain” Answer Two Different Questions
An international health insurance policy is designed around your healthcare coverage. A Spanish visa application, however, is assessed against specific immigration requirements.
Take the Non-Lucrative Visa (NLV), one of the most common residence visas for Americans moving to Spain without working.
The Spanish Ministry of Foreign Affairs currently states that an NLV applicant must provide:
“Original and a copy of the certificate accrediting the public or private health insurance contracted with an insurance entity authorized to operate in Spain.”
It also states that the insurance must cover “all risks insured by the public health system in Spain.” — Spanish Ministry of Foreign Affairs, Non-Lucrative Residence Visa requirements.
That distinction matters. A policy might offer excellent international benefits and include healthcare treatment in Spain, but that alone doesn’t establish that it meets the requirements applicable to your Spanish visa.
So when comparing policies, don’t ask only:
“Does this insurance cover me in Spain?”
Also ask:
“Does this specific policy and its documentation meet the requirements of my Spanish visa?”
Before Buying International Insurance, Check the Visa Requirements First
The safest order is visa first, insurance second.
Before purchasing a policy, identify exactly what your visa—and the Spanish Consulate handling your application—requires.
For example, official Spanish consular guidance for residence visas can require coverage comparable to Spain’s public healthcare system. Some official consular instructions go further and expressly specify conditions such as no copayments, waiting periods or coverage limits, depending on the visa and consular guidance applicable to the application.
That’s why broader international coverage doesn’t necessarily mean better visa compliance.
A policy covering dozens of countries may sound impressive, but the number of countries covered isn’t what Spanish immigration authorities are assessing.
Before purchasing, check:
- Is the insurer authorized to operate in Spain where this is required?
- Does the policy provide the healthcare coverage required for your particular visa?
- Are there copayments, deductibles, waiting periods or coverage limitations that could conflict with the applicable requirements?
- Will the policy cover the required period?
- Can the insurer provide the certificate and supporting documentation required for your application?
This last question is easy to overlook. For example, current U.S. consular guidance for the NLV specifically asks for the original and a copy of the health insurance certificate.
💡Good to know:
Don’t rely only on a policy’s marketing description or the words “international” or “worldwide.” Check the actual policy conditions and the documentation the insurer can issue for your Spanish visa. And always review the current instructions from your Spanish Consulate, as requirements and document presentation can vary depending on the visa and consular jurisdiction.
Think Beyond the Visa: You’ll Actually Be Using This Insurance in Spain
Getting your visa approved matters—but remember what happens next: You’re going to live here.

So visa compliance shouldn’t be the only factor you consider when choosing your health insurance.
Ask practical questions too. What medical network will you have access to in Spain? Will you normally access providers directly, or will you need to pay first and request reimbursement? How easy will it be to find specialists where you plan to live? What assistance will you have when you actually need to use the policy?
The goal isn’t simply to find an insurance policy that works on the day of your visa appointment. It’s to choose appropriate coverage for your immigration requirements and your everyday life in Spain.
