Health Insurance for Americans Moving to Spain
Moving from the United States to Spain means adapting to a different healthcare and immigration system. U.S. medical insurance, Medicare, employer benefits or travel insurance should not be assumed to satisfy the health coverage requirements of a Spanish visa or residence application.
Insurance for Expats helps Americans understand which type of health insurance applies to their move and, when private cover is required, arrange DKV health insurance in Spain with the documentation needed for the immigration process.
Do Americans need private health insurance in Spain?
It depends on the immigration route and on whether the applicant has access to qualifying public healthcare. Many Americans moving to Spain under residence routes such as the Non-Lucrative Visa rely on private health insurance for the application. Digital Nomad applicants can have different healthcare options depending on their Social Security situation.
The key point is that the insurance requirement should be checked against the specific visa or residence route rather than applying one rule to every U.S. citizen moving to Spain.
- Non-Lucrative Visa applicants commonly need qualifying private health insurance
- Digital Nomad applicants may use private insurance or qualifying Social Security coverage depending on their case
- Student and family residence routes have their own healthcare requirements
- U.S. travel insurance should not be assumed to meet Spanish residence requirements
- The policy and certificate should be reviewed for the specific visa or residence procedure
Common insurance routes for Americans moving to Spain
Non-Lucrative Visa: Americans applying for the NLV generally use comprehensive private health insurance in Spain that matches the requirements of the consulate handling the application.
Digital Nomad Visa: private health insurance is not required in every case. Some applicants can rely on Spanish Social Security or an applicable international arrangement. If private insurance is required, the policy must meet the UGE criteria.
Students and family members: the applicable insurance depends on the authorization being requested, the duration of stay and the applicant’s healthcare status.
Because the requirements differ, we recommend confirming the immigration route before buying the policy.
- NLV: review private health insurance requirements before the consular appointment
- DNV: confirm whether private insurance or Social Security coverage applies
- Student applications: verify the specific study authorization requirements
- Families: make sure each applicant is correctly included in the required healthcare evidence
- Residency renewals: keep the policy and supporting documentation consistent with the permit requirements
DKV health insurance for U.S. citizens in Spain
When private health insurance is required, our visa-focused DKV option provides comprehensive private healthcare in Spain without co-payments and without waiting periods for covered services, subject to the policy terms and medical underwriting.
The contract is annual and the application can be prepared using a valid U.S. passport before you have a NIE. The insurer provides policy documentation and an insurance certificate for the immigration process. Basic dental cover is included with the health product. Repatriation is not included in the health policy and can be arranged separately when desired or required for another purpose.
Coverage is available for individuals and families, subject to insurer acceptance.
When should Americans arrange health insurance before moving?
Do not leave the policy until the last minute before your visa appointment. The insurer needs time to process the application, review any medical declaration when required and prepare the documents used in the immigration file.
For the visa-focused DKV product we arrange, the effective date can generally be scheduled up to 90 days in advance. That lets you coordinate the policy start date with your planned move and visa timetable.
Before purchasing, send us your visa or residence route, planned appointment or filing date, intended arrival date in Spain, date of birth and passport details for each applicant.
What if the Spanish visa is denied?
If a visa application is denied before the health policy effective date, the visa-focused product can qualify for a full refund, subject to the insurer’s terms and the required evidence of denial.
If the policy has already started, the refund is generally proportional to the unused part of the annual period rather than to time in which coverage was already active.
The DKV health policy also includes emergency medical assistance abroad of up to €12,000 for trips of up to 90 consecutive days. This benefit does not replace the private medical insurance required for a Spanish residence route when qualifying health cover must be demonstrated.

Health insurance for Americans moving to Spain: FAQ
Can I use my U.S. health insurance for a Spanish visa?
Do not assume that a U.S. health plan will meet the requirements of a Spanish visa or residence application. The acceptable coverage depends on the immigration route and the authority processing the application.
Is travel insurance enough for moving to Spain?
Travel insurance is not a substitute for qualifying private medical insurance when the visa or residence route specifically requires comprehensive health coverage in Spain.
Can I buy Spanish health insurance before I have a NIE?
Yes. For the visa-focused DKV application, we can work with a valid passport before the NIE is issued, subject to the insurer’s requirements.
Can my spouse and children be included?
Yes. Couples and families can be insured, subject to insurer acceptance. Each family member must be correctly reflected in the documentation used for the immigration process.
How long is the policy?
The visa-focused health insurance contract is annual. The effective date should be coordinated with the visa timetable and planned move to Spain.
Does the health policy include repatriation?
No. Repatriation is a separate product. It can be arranged independently if desired or if another procedure specifically requires it.
What is the maximum age for the visa-focused DKV product?
The product is available up to 75 years and 11 months, subject to insurer acceptance and medical underwriting.
What documents will I receive for my visa application?
Once the policy is processed, the insurer provides the policy documentation and insurance certificate used to evidence the health cover. The exact documents should be checked against the requirements of the consulate or immigration authority handling the application.
