Digital Nomad Visa Health Insurance in Spain

Spain’s Digital Nomad Visa and international teleworker residence route have specific healthcare rules. Depending on your Social Security position, you may need private health insurance in Spain — but not every digital nomad applicant needs to buy a separate policy.

Digital Nomad Visa health insurance requirements

The Unidad de Grandes Empresas (UGE), which handles international teleworker residence applications under Law 14/2013, distinguishes between applicants who already have qualifying healthcare coverage and those who need private insurance.

If private insurance is required, the UGE states that the policy must provide coverage comparable to Spain’s National Health System. It does not accept travel insurance, insurance based only on reimbursement of medical expenses, policies with co-payments, or policies with waiting periods. When does a digital nomad need private health insurance?

Private health insurance is commonly needed when the applicant cannot prove qualifying healthcare coverage through the Spanish Social Security system or through an applicable international Social Security coordination arrangement.

If an international Social Security agreement allows the applicant to remain covered in the country of origin, the applicant must provide the relevant certificate. Where that arrangement does not include healthcare in Spain, the UGE requires health insurance that covers that contingency.

By contrast, when the applicant will register with Spanish Social Security after the residence authorization is granted, the UGE documentation states that separate health insurance evidence is not required for that reason. This is why the insurance decision should be made only after reviewing the applicant’s Social Security route.

DKV health insurance for Digital Nomad Visa applicants

When private medical insurance is required, our visa-focused DKV option is designed to satisfy the characteristics the UGE expects from private coverage: comprehensive private healthcare in Spain, no co-payments and no waiting periods for covered services, subject to the policy terms and medical underwriting.

The contract is annual and can be prepared using a valid passport before the applicant has obtained a NIE. Policy documentation and an insurance certificate are issued for the immigration file. Basic dental cover is included with the health product. Repatriation is separate from the health policy and can be arranged independently if desired.

Because the Digital Nomad route is closely linked to Social Security, we recommend confirming first whether you genuinely need private insurance before purchasing it.

When should I arrange DNV health insurance?

If your case requires private insurance, do not leave the policy until the day before filing. The application must be processed, any medical declaration reviewed where applicable, and the insurer documentation prepared for the residence or visa file.

For the visa-focused DKV product we arrange, the effective date can generally be scheduled up to 90 days in advance. This allows the start date to be coordinated with your move to Spain and the timing of the application.

What happens if the Digital Nomad application is denied?

If you purchase the visa-focused health policy and the immigration application is denied before the policy effective date, the insurer’s visa product can qualify for a full refund, subject to the policy terms and evidence of the denial.

If the policy has already started, the refund is generally proportional to the unused period rather than to months in which coverage was already active. We explain the timing and refund conditions before the policy is issued so you can choose the effective date carefully.

The DKV health policy also includes emergency medical assistance abroad of up to €12,000 for trips of up to 90 consecutive days. This travel assistance is not a substitute for the private medical insurance required by the UGE when qualifying health cover must be demonstrated.
