A health insurance waiting period is the time between the policy start date and when certain medical services can be used. For expats in Spain, waiting periods matter both for everyday healthcare and, in some cases, for visa or residence applications.

Many services may be available from the beginning, while some higher-cost treatments, hospital services or advanced diagnostic procedures can have specific waiting periods depending on the insurer and product.

What is a waiting period in health insurance?

A waiting period is a contractual delay that applies to specific benefits after a health policy becomes effective. The exact periods depend on the insurer, product and current policy conditions.

DKV explains that many services can be used from the first day, but some benefits have specific waiting periods. According to DKV’s current public information, examples can include high-technology diagnostic tests, some surgical procedures, hospitalization, childbirth and transplants.

Examples of DKV waiting periods

  • High-technology diagnostic tests: 6 months.
  • Ambulatory surgical procedures: 6 months.
  • Surgery, hospitalization and surgical prostheses: 8 months, subject to the applicable policy conditions and exceptions.
  • Childbirth: 8 months, with the exceptions described by DKV.
  • Transplants: 12 months.

These periods can vary by product or additional cover, so the contractual wording in force at the time of purchase should always be reviewed.

Waiting periods and Spanish visa applications

Visa and residence requirements are separate from ordinary health-insurance use. Some immigration procedures require qualifying health coverage without waiting periods or copayments.

For international teleworker residence applications, the Unidad de Grandes Empresas (UGE) states that, when private insurance is required, travel insurance, reimbursement-only policies, policies with copayments and policies with waiting periods are not accepted. Whether private insurance is required at all depends on the applicant’s Social Security and healthcare situation.

Can a waiting period be waived?

Some insurers may consider waiving or modifying waiting periods in specific circumstances, such as when a person moves from another policy with comparable coverage. This is not automatic and depends on the insurer’s underwriting rules, previous cover, continuity between policies and the documentation provided.
